Employees and Plan Participants

You may request a distribution from a 401(k), Profit Sharing Plan, Defined Benefit Plan, 403(b) or 457 Plan
once you have Retired or Separated from Service of the Plan Sponsor. Timing of this distribution depends on
the operating legal document for your specific plan. Your balance/benefits may not be immediately available.
Please review your Summary Plan Description.

If you are eligible to request a distribution at this time, please click on the link below, complete the form, and
fax or email the form to BAC. Distributions that are available immediately take no less than 10 business days
to process.

INFORMATION ABOUT PARTICIPANT LOANS

Your plan may allow for you to receive a loan from your vested account balance. Generally, you may receive 50% of your vested
account balance up to a maximum of $50,000. Check your Summary Plan Description to determine if you can apply for a loan.

Loans are paid back to the plan via payroll deduction to your account with interest. Check the PRIME LENDING RATE and estimate
your loan using the calculator below. You can only amortize your loan over a maximum of 60 months. BAC will charge your $200 to
set up your loan. No annual fee applies. The set up charge can be taken from your account balance.
